Medical Information Pathways
 

When personal medical information is used for treatment purposes, it may follow the pathway below.

1. Patient medical record is accessed by doctor and staff.

2. Prescription provided via fax or phone to pharmacist.

3. Pharmacist adds to or creates patient file.

4. Pharmacist submits bill to insurance for reimbursement.

5. Doctor submits bill to insurance for reimbursement

6. Insurance pays pharmacy

7. Insurance pays doctor

8. Insurance sends notice of amount paid or denied to patient

9. Others who may review medical information are financial auditors, hospital quality control, computer support personnel, medical malpractice lawyers.

When personal medical information is used for non-treatment purposes, it may follow the pathway below.

1. Person applies for non-group health insurance, employment, or life insurance.

2. Person signs release allowing access to PHI.

3. Physical examination and medical history are taken. Prior records may also be requested.

4. Record of examination is sent to requestor and Medical Information Bureau (MIB)

5. Person may ask MIB or doctor what is in their file and who requested information.

6. Decision made to employ or insure person.
